I understand individual,
have applied for enrollment in a woodcarving course the to be conducted at Long
Island School of Classical &
Woodworking by Leonid Zakurdayev.
I understand that woodworking can be dangerous activity which could result
in damage to property, or serious injury to me. As an enrollee, I completely
and fully assume100% of or damage or
injury as a result of activities engaged in and/or related to enrollment and
participation in any woodworking course
at Long Island School of Classical Woodcarving & Woodworking. In
consideration of the permission extended to
me to enroll in a course conducted by Leonid Zakurdayev, and with intent to be
legally bound, I hereby assume full
responsibility for all injury or damage to me or my property , including that
caused by my negligence of others,
arising from related in any way to participation in said course, and for
myself,my heirs, executors, administrators
and assigns forever release Leonid Zakurdayev and Long Island School of
Classical Woodcarving &
Woodworking, its shareholders, officers directors ,agents, employees, teachers
and landlords from all liability,claims,demands,
actions cause or action,of whatever type or nature, for injury to my
person(including death)or my property,
arising from or related in any way to enrollment or participation in said
course.
